The Struggles and Challenges of PCOS
Despite Xiaomei from the Philippines being young, fit, and healthy, she was still diagnosed with PCOS. Xiaomei's husband is a soldier, and unlike most husbands who go home for dinner every night, her husband only returns home once every ten days or half a month due to work demands. Xiao Mei is married in Taiwan and longs for a baby for additional companionship in her daily life.

After multiple failed attempts, Xiaomei decided to visit NUWA Fertility Center for professional medical help and with high hopes of achieving pregnancy despite all her challenges with PCOS. Xiaomei finally met with Dr. Pei-Chen Huang for initial consultation.

During the first consultation, Dr. Huang prescribed ovulation drugs for two months. Although Xiaomei’s husband was willing to accompany her for follow-up visits, the timing for her follicle tracking didn’t align with her husband’s days off, which left her feeling frustrated.

She managed to endure the injections, medication, and blood tests, but the most painful part was the emotional struggle. During the sensitive period of dealing with infertility, every word from family members felt like added pressure. When negative emotions arose, her husband wasn’t there to support her. We are grateful she shared her experiences with us.

Being a military spouse, she had to be very independent, especially living alone. Most of the time, she had to rely on herself, with her husband being like a weekend roommate. The toughest times were when she got sick—going to the doctor alone and dealing with both physical discomfort and psychological loneliness was a real challenge.

The upside was that she had a lot of freedom during the week, like a pseudo-single lifestyle. She could sleep in a double bed alone, leave house chores undone without anyone nagging, and look forward to her husband’s visits on Friday nights. Weekends were precious as they cherished their time together. Although they spoke on the phone daily, some things were just better discussed face-to-face.

PCOS Diagnosis During Pre-Marital Checkup Led to IVF Decision with Integrative Medicine Support

She knew she had polycystic ovary syndrome (PCOS) from her pre-marital checkup, and with the difficulties of being a weekend couple, the process was tough. Unfortunately, things didn’t go as planned, and they decided to go for IVF. She hesitated, considering the financial and emotional burden. Her husband reassured her by setting a personal limit, giving her the courage to proceed. She took the medication on time, administered her injections, and had so many blood draws that her veins hardened, making her ask, “Can you take it from the back of my hand instead?” She even had to get an IV drip due to ovarian hyperstimulation syndrome after egg retrieval. Despite all the physical pain, she said, “Does it hurt?” It wasn’t too bad.

For her, who wasn’t afraid of injections or blood draws, the real pain was the emotional struggle. Facing all the physical discomfort alone, she didn’t dare share all her emotions with her husband, holding back whenever possible. The negative emotions were overwhelming. Ms. Chen's Journey: From Irregular Periods to Weight Loss Success

Ms. Chen initially sought treatment due to her irregular menstrual cycles, often needing medication to induce her periods. After visiting an obstetrics and gynecology clinic, she was diagnosed with polycystic ovary syndrome (PCOS) and was advised to lose weight.

Xiaowen’s Journey: Overcoming Failed IUI and IVF Attempts with Chinese Medicine and Lifestyle Changes

Xiaowen came to see me in April 2021 after her first IUI treatment had failed. Her AMH was 6.53, indicating a tendency toward polycystic ovary syndrome (PCOS), and she did not respond well to Western medications. When she first came to me, she was overweight, so we began focusing on improving egg quality and weight loss while also continuing with her Western medical treatments.

Two Failed IUIs and an Unsuccessful IVF Cycle

After another unsuccessful IUI attempt in April, they decided to proceed with IVF in May. On the day of the egg retrieval, 14 eggs were retrieved. However, after fertilization and development to day five, only three embryos remained. Genetic testing revealed that two had genetic abnormalities and one was a high-mosaic embryo. The result of this cycle was disheartening: no embryos were recommended for transfer.

This was a huge blow to Xiaowen, shattering her confidence. However, she decided to take it slow and focus on nurturing her body, planning to start the next treatment after six months of serious preparation.

Couple’s Joint Effort: Weight Loss, Timely Herbal Medicine Intake, and Regulating Lifestyle

During this preparation period, basal body temperature tracking revealed that her luteal phase only lasted 11-12 days, indicating poor luteal function. Gradually, she started to lose weight. Though it seemed like only a little at each check-up, over six months she managed to go from 68 kg to 61 kg.

I suspected she had conceived naturally and encouraged her to take a pregnancy test. Xiaowen, however, was reluctant, fearing disappointment, so she delayed it. Even after confirming the pregnancy, she was hesitant to go for an ultrasound due to a lack of confidence, worrying something might be wrong. During early pregnancy, she did experience occasional dark brown spotting, so I continued to prescribe herbal medicine to support her pregnancy.

Ultrasound Reveals a 6-Week Embryo: Relaxing and Following the Treatment Led to Success

It wasn’t until May that she told me she had finally gone for an ultrasound, and the embryo was already six weeks old, with a heartbeat detected on the same day! Both she and her husband were overwhelmed with emotion, with her husband almost in tears. Xiaowen, still lacking confidence, was cautious about celebrating too early, aware that about 30% of natural pregnancies end in miscarriage during the early stages. By mid-June, at around 11 weeks, she started to feel a bit more secure.

Her pregnancy, though not without its challenges—including an infection and a Covid-19 diagnosis—went smoothly without other complications, much to her relief. Now 32 weeks along and with everything looking normal, Xiaowen came to see me specifically for a photo, saying she wanted to let me post a belated "graduation announcement." She also gave me a heartfelt card filled with words of gratitude, which moved me deeply.

She mentioned that, besides taking her medicine and adjusting her lifestyle diligently, she also made a habit of soaking her feet regularly. When I asked if she had any other tips to share, she said:

“Although it might not be helpful to say this, staying relaxed is important. Follow the treatment plan, let your body prepare silently, and when you’re too busy with life to worry about pregnancy, that’s when it happens! You need confidence, patience, and peace of mind.”

Isn’t the success rate for IVF high for those under 30? Even if the first attempt fails, the second should work, right? With over 30 eggs retrieved, getting pregnant should be easy, and having a second child shouldn’t be a problem either, right?

PCOS, four years of trying to conceive, and two unsuccessful IVF transfers.

For H, the outcome hasn’t been as easy and beautiful as others might think.

Married before 25, H thought her youth would guarantee quick success. However, after waiting 3-4 years with no progress, she decided to pursue IVF treatment. With potential PCOS, she retrieved 24 eggs, resulting in 9 embryos ready for transfer.

The first transfer failed, possibly just bad luck. The second attempt also didn’t work—maybe it was just statistics. But after transferring all remaining embryos with no signs of pregnancy, she and her husband began to question what was happening.

After getting married last year, she went to a clinic for a check-up, and the doctor informed her that blood tests and ultrasounds showed no issues, recommending medication to induce menstruation.

Could I have Polycystic Ovary Syndrome (PCOS)?

C found it strange that if there were no problems, why was her menstruation so irregular? She traveled from Yunlin to Taichung to seek my help in regulating her cycle. During the consultation, I discovered that C often stayed up very late for work (until 3 AM), which contributed to her high-stress levels, causing issues like mouth sores and acne.

I suggested checking her ovarian function, and the results showed her ovarian function index was too high (AMH: 11 ng/ml), confirming that C likely has PCOS. She was surprised, thinking, “I’m not overweight (BMI: 22). How could I have PCOS?”

How is Polycystic Ovary Syndrome Diagnosed?

According to the commonly used 2003 Rotterdam criteria, two of the following three criteria must be met:

Irregular menstruation.
Elevated androgen levels (e.g., hirsutism, acne, male pattern baldness).
Ultrasound showing polycystic ovaries.
AMH has recently become another useful tool for accurately diagnosing PCOS. Normal values are approximately 2-6; values greater than 6 along with clinical symptoms can also indicate PCOS.

Common Questions about Polycystic Ovary Syndrome

Q: Do all PCOS patients need to lose weight?

Not necessarily. Only those with a high BMI or waist circumference need to focus on weight loss. Studies show that, unlike the overweight patients in Europe and America, half of the PCOS patients in Taiwan are of normal weight or even underweight. For thin patients with PCOS, weight loss has little effect on improving menstruation. However, not needing to lose weight doesn’t mean they shouldn’t exercise!

Doctor’s Summary: Exercise helps regulate hormones and improve menstrual cycles for both overweight and underweight PCOS patients.

Q: How long does it take to regulate menstruation with Traditional Chinese Medicine (TCM)?

Typically, 3-6 months. For PCOS patients, the lack of menstruation is due to not ovulating, not because menstrual blood is trapped in the uterus.

Doctor’s Summary: TCM aims to improve the body's constitution to help establish regular ovulation. Once regular ovulation is achieved, menstruation will also regulate itself after stopping medication (this differs from the concept of using Western medications to induce menstruation or ovulation).
